Italian football is built on rivalries shaped by geography, identity and success, but few match the scale and uniqueness of AC Milan versus Inter Milan.
This is a fixture defined not only by trophies and titles, but by coexistence. The two clubs share the same city, the same skyline and the same stadium – yet remain divided by colour, culture and history. The Milan Derby rarely fails to meet expectations, consistently producing drama, tension and unforgettable moments under the lights.
Known as the Derby della Madonnina, the rivalry takes its name from the golden statue of the Virgin Mary that stands atop the Duomo di Milano. Affectionately called “La Madonnina” by locals, the monument symbolises the city's unity, a unity that disappears for 90 minutes on derby day.
From its origins in internal disagreements to the European nights that shaped football history, this rivalry has defined Milan for over a century. Here, Football Ground Guide takes a look at the origin, history and head-to-head record of one of the most iconic derbies in world football.
History and origin of the Derby della Madonnina

The rivalry dates back to the early 20th century and was born from internal disagreement rather than geography.
AC Milan were founded in 1899 by English expatriates under the name Milan Cricket and Football Club. They quickly established themselves as one of Italy’s earliest powerhouses, winning league titles in 1901, 1906 and 1907.
However, in 1908, tensions arose within the club regarding the signing of foreign players. A faction broke away in protest and formed a new club – Internazionale with a more open stance toward international talent.
That split did not just create another football team; it created a rivalry that would shape Italian football for generations.
The first recorded meeting between the two sides took place in January 1909 in the Prima Categoria, the forerunner to Serie A, with Milan winning 3-2.
In the early decades, the clubs were seen as representing different social classes. Inter were often associated with Milan’s bourgeoisie, while Milan were linked to the city’s working class. Inter supporters earned the nickname “motoretta” for arriving at matches on motorcycles, while Milan fans were dubbed “tramvee”, referencing public transport. Today, that divide has largely faded, replaced by personal loyalty and family tradition.
Inter enjoyed strong dominance in the late 1920s and 1930s, claiming multiple league titles, including the first official Serie A championship in 1929-30. During that period, the legendary Giuseppe Meazza became a defining figure in the rivalry.
Milan would later reassert themselves, beginning a new era of success in the 1950s and beyond, ensuring that neither club could permanently claim supremacy.
Head-to-head record between Ac Milan and Inter Milan

Across more than a century of competition, AC Milan and Inter Milan have faced each other 245 times in official matches.
- Inter wins: 91
- AC Milan wins: 83
- Draws: 71
The balance has shifted repeatedly across eras, with both sides enjoying spells of superiority.
The most recent meeting between the two came on 23 November 2025, when AC Milan secured a narrow 1-0 victory over Inter.
In terms of overall trophies, AC Milan hold a slight edge with 50 major honours compared to Inter’s 46. However, Inter boast 20 Serie A titles – one more than Milan’s 19 – underlining just how finely balanced their success has been over the decades.
